Sodium Chloride Runoff Damage Report Form
In compliance with Public Act 23-31, Section 19, Connecticut Health Departments and Districts are required to establish an electronic reporting system for homeowners to report damage to homes or wells caused by sodium chloride run-off. Beginning January 1, 2024, these reports will be submitted annually to the Connecticut Office of Policy and Management (OPM). The information you provide will help OPM evaluate the need for potential financial remediation.
