Heart Screening & Intervention for CBCCEDP Participants
- You are eligible to be screened for the WiseWoman Program if you are enrolled in the Connecticut Breast and Cervical Cancer Program - received a Breast exam, Pap test or Mammogram and are 35 to 64 years old.
- If you are found to be at risk for heart disease - you will be eligible to participate in nutrition and physical activity interventions to help decrease your risk of heart disease.
