• Medical Screenings: Blood Pressure, Blood Sugar, Urine Pregnancy

• Lab tests: A1C, Cholesterol, Hepatitis C, Hemoglobin, HIV

• Consultation with medical staff, Referrals, Follow-up Care

• Vaccinations (Tdap, Covid-Pfizer, Shingles, Pneumonia, Hep B)

• And Much More

UCFS & Generations Community Health Worker to assist with

• Access to Primary Care (Medical, Dental and Behavioral Health)

• Assistance with insurance applications

• Assistance with linkages to financial benefits and community resources

• Assess additional needs and refer as needed

-Open to all ages. Insurance accepted but not required
