The 2016 CHA describes the health status of the population of the communities UHD serves, including identifying areas for health improvement, determining factors that contribute to health issues, and identifying assets and resources that can be mobilized to address population health improvement. This assessment was developed through a participatory, collaborative process with diverse sectors of the community.
